What You Need to Know:

INTENT & PARTICIPATION REQUIREMENTS

• Marching Band is a highly physical activity and student health is our highest priority.

• All students intending to participate as a performing member of the Union H.S. Renegade Regiment must complete an annual physical exam.

DOCUMENTATION SPECIFICS

• The Union Bands exam form must be used for the examination, and must be signed by the evaluating physician. Generic OSSAA forms available at your Physician's office are not valid and will be rejected.  Forms coming soon. → ENGLISH Form | SPANISH Form

• Exams must be dated on or after April 26, 2024.

• Exams are valid until April 30, 2025.

• Exams must be completed & submitted, showing the student as cleared to participate by May 16, 2024 (the first day of full Spring Training).

• Please retain the original form documentation for your records.

EXAMINATION

• Union’s Physical Exam event is held annually at the UMAC, scheduled for the evening of Monday, May 1, 6-8pm. We suggest arriving early to avoid long lines.

  • The fee for this service is $20.00.

    • Cash or check, or pre-pay online by April 30.

    • Insurance is NOT required to receive this service.

    • Charms account funds are not available for paying this fee.

  • Exams are conducted by Advanced Orthopedics of Oklahoma.

• If you prefer to have your Primary Care Physician or an Urgent Care provide the Physical Exam, we suggest contacting their facility to schedule an appointment as soon as possible. 

  • NOTE: if the student has recently completed an annual child well-check with his/her pediatrician, it’s likely that the physician will retroactively complete the physical exam form based on the recent visit.

  • Access Medical Urgent Care – 81st & Mingo: Approx. $25.00

  •  St. John’s Urgent Care – 81st & Memorial: Approx. $30.00

  •  My Doctor Now – 71st & Yale: Approx. $25.00

OTHER INFO

• Students participating in multiple activities / sports need to complete only one exam. If participating also in athletics, please plan to submit a copy of your completed exam to the coach in his/her preferred format.
